Welcome to Slate’s Patient Portal!

Our patient portal allows clients to easily review and pay their billing statements online.

We utilize Procentive’s online patient portal services.

Your therapist will discuss our online portal at the beginning of your treatment and with your consent, our administrative specialist will email you the link to set up your username and password. The initial link to your account expires after 24-hours, so we ask that when you receive the link, you follow the easy steps to set up your account. 

If you need to request access to the portal, need the initial portal link resent, or have any questions about the portal or a billing statement, please give us a call at 952-926-2526 and our administrative specialist would be happy to assist you.
